How do I get started with Hormone Replacement Therapy?

1. This process includes the submission of pertinent medical information to our staff.

2. This personal information abides by all privacy laws.

​

3. Download and complete HRT form above. There is an initial online consulting fee charged and returned as a credit if therapy is initiated at first fill.

​

4. Upon receipt and review of hormone replacement questionnaire, our Pharmacist will offer a "road map" sample prescription and at your request will be given to your health provider to assist with your care.

​

5. Once your provider agrees with our suggestion and signs your prescription we will be happy to assist with you care. They may phone or fax the pharmacy.
