Service Description

Pharmacist will meet with you to discuss hormone questionnaire information, discuss options, goals,medical history, and symptoms. Once performed, client will receive a plan for your provider to utilize as a step plan for your prescription. Client will receive a credit for evaluation on first prescription if filled within 14 days. Note that controlled substances are not allowed in credit by the state of Louisiana.

Cancellation Policy

To cancel or reschedule client must contact us 48 hours (2 business days) prior to consultation. Refunds will not be given unless contacted
