Hormone evaluation via Zoom

Service Description

Personalized evaluation of hormone symptoms, goals, medical conditions and provider list. Patient must download and fill out Hormone questionnaire form and provide it to Pharmacist prior to consult. At completion, client will be provided a step plan to give to provider to suggest therapy and obtain signature for therapy. Client will receive $30 credit on first prescription if utilized within 14 days of consult. Please note that controlled substances are not allowed monetary credit in Louisiana on prescriptions ****

Cancellation Policy

To cancel or reschedule client must contact us 48 hours (2 business days) prior to consultation. Refunds will not be given unless contacted
